    Mail-In Forms are available at Toyota dealerships until June 30, 2008 or online at www.toyotaownersonline.com/tirepromo08 through July 7, 2008.Return to top
  2. Why should I make a copy of my Mail-In Form and Service Invoice before I send them?
    Once you submit your paperwork and it is received by Toyota, your original paperwork will not be returned to you. It is always a good idea to keep a copy for your reference should there be any questions regarding your gas card submission. It is very important that you keep a copy of your gas card documents in the case that your submission gets lost in the mail. You will need this information to have your request processed.Return to top
  3. I lost my Service Invoice. What should I do?
    Contact your Toyota dealership where you made your tire purchase. A duplicate Service Invoice or Repair Order may be provided. You must provide a Service Invoice to qualify for a gas card. Credit card receipts will not be accepted unless accompanied with a Service Invoice.Return to top
  4. Can I request a gas card over the phone?
    Unfortunately, not for this promotion. To qualify for a gas card, you must submit all of the required documents by mail. This is to verify that the 4 tires were purchased within the valid promotional dates.Return to top
  5. I missed the cut-off date for submitting the paperwork for the gas card. If I send it in late, will I still receive the gas card?
    Unfortunately, you will be ineligible for the rebate if the postmark date has passed. All forms must be postmarked by July 7, 2008 and received by Toyota no later than July 14, 2008.Return to top

    Gas cards will be mailed 8-10 weeks after the promotion end date. You may receive your card sooner than this — to check the status of your gas card, please use this website and enter your last name and tracking number that was emailed to you.Return to top

    If you included a valid email address on your Mail-In Form, a unique tracking number was sent to that email address. If you have not received a tracking number, and more than five weeks have passed since you submitted your paperwork, please go to the Contact Us section on this site and request a tracking number.Return to top

    You can check the status of your gas card by visiting tires.toyotapartsandservice.com and entering the unique tracking number that was emailed to you. Please allow up to 3-5 weeks from the date you mailed your paperwork for information to appear in the gas card tracking system.Return to top

    Please ensure that the last name you enter is the same last name that you included on your Mail-In Form. If you still encounter problems, please use the Contact Us form to submit a request.Return to top
  10. The tracking system states that my gas card has been mailed. However, the estimated delivery date has passed. What should I do?
    Please double check the name and address that appears when you check the status of your gas card. If any of this information is incorrect, you will need to call American Express® Customer Service at (800) 575-7365. You will need to provide them with the name and address that the gas card was sent to (even if this is incorrect). They will be able to cancel the original gas card and reissue a new card to a revised name and/or address. If you have not received your gas card but the information listed on this website is correct, please call American Express® Customer Service at (800) 575-7365. You will need to provide them with your name and address. They will cancel the original gas card and reissue a new card to you. NOTE: American Express® Customer Service can only provide assistance to customers whose gas cards have already been mailed. When you check the status of your gas card on this website, if your gas card has not been sent and you have a promotion question, please use the Contact Us form to submit any questions.     If your gas card request has been rejected, it means that all of the requirements of the offer were not met. You will receive a letter in the mail outlining which requirements were not met, along with the original paperwork that you submitted. Please read the letter carefully and be sure to return all the required items in the time specified to qualify for a gas card. Please be sure to make copies of all documents for your records.
